A personalized 12-week program for Central Illinois adults who are ready to stop keeping an eye on their health and start changing its trajectory. Built and guided by a physician assistant and a Doctor of Physical Therapy.
Application only. We follow up personally to talk it through. No obligation.
You have watched a parent or a sibling go through it. You would rather understand your own numbers now than find out the hard way later.
The habits that used to work stopped working. The weight settles differently, recovery takes longer, and nothing you have tried has moved it.
Cholesterol, blood sugar, A1c. Close enough to matter, not far enough for anyone to hand you a plan. So it just sits there.
Meaningful data, expert guidance, and a clear plan. Not another generic fitness challenge or quick fix.
Being told to keep an eye on it is not a plan. You deserve to know where your health stands today, where it is heading, and what to do about it now.
Nothing starts until we know where you are standing today.
We measure where your health and your movement stand today.
You learn what your results mean and where they are heading.
You follow your blueprint for how you eat, move, train, and age.
We measure what changed and build the next stage from it.

No. It is about twenty minutes to understand your goals and answer your questions. If Longevity Foundations is not the right fit for you, we will say so.
No. The program begins with a baseline of where you are right now, whatever that looks like. The training is coached in person and adjusted to your body, which is the reason a physical therapist runs that side of it.
Neither. Longevity Foundations is a health and fitness program led by clinicians, and it is not a substitute for individual medical care from your primary care provider. We would encourage you to keep your own physician in the loop, particularly if you have an existing condition or take medication.
On the movement side: how well your joints move, your strength for everyday tasks, and your balance. On the metabolic side: how your body is handling blood sugar and weight, your cardiovascular fitness, and how your glucose responds to food, movement, stress, and sleep.
You reassess with both clinicians and see what changed. From there we talk through what makes sense next. Nothing continues automatically without that conversation.
This is the founding round, and the program is coached in person by two clinicians. Fifteen is the number we can take without diluting the attention each person gets. We use an application so we can make sure the program is a genuine fit before anyone commits. Applications close once the fifteen spots are filled.
Tell us where you are today. We follow up personally to talk it through and see whether this is the right fit for you.
Takes a minute. We will reach out from there.
